On May 7, 2019, a FREE 11th grade REQUIRED Meningococcal vaccine (needed for entry into 12th grade) will be offered to students at the West Carrollton High School. Below are links to the information sheet and the consent form (in English and Spanish).
Forms are due in the clinic no later than April 26, 2019.
