Below are links to the application documents. The Parent Agreement and the High School Credit Requirements page, if applicable, need to be turned in with the application.
Applications need to be submitted by the prospective student's legal guardian. Please submit to the Board of Education Office, 430 East Pease Avenue. The office will accept applications from 8:30 am - 4:00 pm Monday through Friday. Questions regarding the open enrollment process can be directed to the Board of Education Office at (937) 859-5121. Face masks are required for entry into the building.
Parents may submit applications by either method below:
- E-mail the completed application and required proof of residence to [email protected]
- Deliver the completed application and a copy of the required proof of residence to the West Carrollton Board of Education Office. If after office hours, place all of your documents in a sealed envelope and leave it in the red box outside the entrance to the Board of Education office.
Applications will also be available in the red box outside of the entrance to the Board of Education Office located at 430 E. Pease Avenue.
The application and proof of residence must be received on or before May 28, 2021 - late applications will not be accepted.
